Your first visit is designed to give you the time and space to tell your full story. We’ll review your medical history, prior labs, and any past testing or treatments, and work together to better understand your experience with urinary symptoms.
You’ll learn about our approach to UTI care and how it can be individualized to best support your health and healing. We’ll also discuss our urine testing methods, and you’ll be provided with a take-home kit so you can easily collect and mail urine samples for PCR testing.
We do not perform a physical exam during the first visit. Instead, your appointment takes place in a consultation room rather than an exam room, allowing for a thoughtful, conversation-based visit focused on listening, education, and collaborative planning.

Antibiotics can be an important and appropriate part of treatment in certain situations, and we use them thoughtfully when indicated. At the same time, our goal is to move away from reflexive or repetitive antibiotic use whenever possible.
We focus on preventive and restorative strategies that support long-term urinary health, including individualized care plans that may incorporate non-antibiotic therapies such as herbal remedies, microbiome support, and lifestyle-based interventions. We are also continuously evolving our approach and developing new treatment modalities as research and clinical experience expand our understanding of chronic UTIs.

The bladder and vagina each have their own communities of bacteria, called microbiomes. Many of the same types of bacteria live in both areas, but the balance of these bacteria can differ. Factors like age and hormones can change the vaginal microbiome, which can then influence the bacteria found in the bladder.
When these microbial communities are healthy and balanced, they help protect against infection and support normal immune function. When the balance is disrupted, the body may be less able to fight off infections or may stay stuck in an inflamed, irritated state. Over time, this can contribute to frequent UTIs or ongoing urinary symptoms.

Standard urine cultures have long been used to diagnose UTIs, but they have important limitations. They are designed to detect a narrow group of fast-growing bacteria and often report only a single “dominant” organism. This means they can miss infections involving multiple bacteria, slow-growing microbes, or bacteria living in biofilms. As a result, cultures may come back negative or incomplete even when symptoms are ongoing.
We also use urine DNA (PCR) testing, which looks for microbial DNA directly in the urine rather than relying on bacterial growth. This testing allows for detection of a broader range of organisms. Because this testing is very sensitive, results are always interpreted carefully and in the context of your symptoms.

Yes — many of our patients live outside the Portland area. We offer both in-person and virtual visits for patients located in Oregon, as well as virtual appointments for those in California, Washington, and Wisconsin. You are always welcome to travel to see us in person, and we’re honored when patients choose to do so.
